Your First Visit

On your first visit, when you enter the office, you will be asked
to complete some paperwork including an extensive health history form. It’s very important that Dr.
Porter find out as much information about your current and previous health conditions so that he can make an accurate diagnosis.
Please bring any pertinent information with you such as MRI reports or X-rays. And, if you are currently
taking any medications, we’ll need a complete list.
Once
your paperwork has been completed, you can expect to have a conversation with one of our staff members who will review your
paperwork and answer any questions you may have. Then, you will meet with Dr. Porter and discuss your condition
and what’s bothering you. He will ask you to do some simple range-of-motion exercises to help him evaluate your health.
After examining your posture, he may take some x-rays if necessary.
At that point, he’ll talk to you about
what he has found and what he recommends for treatment. If you’re a candidate for chiropractic care, he can perform
your first adjustment on this visit.
If you’re not a candidate for chiropractic care, he will recommend to you some of the top doctors in our area who
specialize in the care you need.
Dr. Porter’s practice is fee for service
and payment is due at the time of treatment.
Cash, check, debit or credit cards are accepted.
When
possible, we will call to verify benefits on your insurance. However, the benefits quoted to us by your
insurance carrier are not a guarantee of payment. If Dr. Porter participates with your
insurance plan, payment will be due by you at the time of service for any non-covered services, deductibles
or co-pays. For patients’ convenience, we promptly bill all insurance carriers weekly.
If you are covered by an insurance plan that Dr. Porter does not participate with, payment in full is due at time of
treatment; rest assured that billings are done at no charge and all possible reimbursements will be sent directly from the
insurance carrier to you.
